The Harlequin Crest drops from level 85 or higher enemies in World Tier 4. To unlock World Tier 4, complete the Fallen Temple Capstone Dungeon in World Tier 3 and switch your difficulty at the World Tier Statue in Kyovashad or the main menu.
The Harlequin Crest has an extremely low drop rate, but farming Cannibals and Cultist enemies, particularly in the Charnel House Dungeon, can slightly improve your odds. Helltide events, other Nightmare Dungeons, and World Bosses are also viable options.
Duriel, the Uber boss introduced during the Season of Blood, has a higher chance to drop Uniques and Uber Uniques, including the Harlequin Crest. To summon Duriel, gather Mucus-Slick Eggs from defeating the Echo of Varshan and Shards of Agony from killing Grigoire in World Tier 4.
